Year 7 Parent/Carer Consent Forms

Welcome to Blythe Bridge High School & Sixth Form. We are making preparations for our new Year 7 students and as part of these we need to gather lots of information in order for your child to have the best start to school in September 2023.

Please read through each of the forms below and answer them as fully as possible. For every form you complete, there is a downloadable copy of the information so that you have a copy for your records.
